LEASE. The agency is currently in a 300-square-foot office with a current lease of $500 per month. The buyer will have the option of relocating the agency to a new location or keeping the existing location. The seller will be leaving the FFE in the current location as part of the sale. *
CENSUS. At the time of the listing, the seller has +/-5 active patients. *
MANAGED CARE CONTRACTS: The agency has managed care contracts, which include: Cal Optima, Humana, and Blue Shield. *
SOFTWARE. The agency is currently using Data Soft Logic as its software platform. *

TRANSITION. The Seller will assist the Buyer will all of the required paperwork and guidance needed to understand the agency’s existing infrastructure.
LIENS/JUDGEMENTS. The seller discloses that to the best of their knowledge, the business has no liens, or judgments against it. It is highly recommended to all buyers to conduct a UCC1 search, and background search on the seller(s), and obtain a good entity standing certificate as part of their due diligence. *
CONSULTANT. The seller can provide the buyer with a list of qualified Medicare consultants that can help the new owner with a Change of Location (CHOL) (as applicable) and the Change of Ownership (CHOW) paperwork.
ALL potential buyers must: (1) Sign an NDA, (2) provide Proof of Funds (redacted), (3) provide a Buyer Profile with a Buyer Representation for this one business and (4) understand that no direct communication with the Seller or the clinical or administrative staff is expected as a part of the pre-due diligence phase of the sale.
COOPERATING BROKERS/AGENTS. We will establish a Co-Op Agreement with licensees, though we do request for all inquiring cooperating licensees to have healthcare business sales experience and knowledge.
