The agency is owned by an RN who is willing to work with the buyer during the transition. The agency does not have any billing issues including no ADRs. The owner obtained the agency’s initial CDPH licensing in May of 2020 with the ACHC accreditation status good until February 2025! *
MOTIVATION. The motivation for the sale is that the Seller would like to slow down in their lives. *
TRANSITION. The owner operators understand the mechanics of running the day-to-day operations of the home health agency and they are willing to work with the buyer for a negotiated period of time during which training, and assistance will be provided. *
LEASE. The lease is currently $975.00 per month, utilities are paid separately by the agency owner. Agency has an active lease in place for one year, expiring in August 2024. The agency seller is requesting for the buyer to remain in the current location as a part of the sale. The seller will be leaving the FFE in the current location as part of the sale. *
CENSUS. At the time of the listing, the seller has three (3) active patients. The existing patients will be a part of the sale of the agency. *
SOFTWARE. The agency is currently using Alora as its software platform. *

TERMS. This listing is not open to financing options (no seller-financing). This is a cash only listing. If Buyer opts to use an escrow, Buyer to assume all escrow costs.
LIENS/JUDGEMENTS. The seller discloses that to the best of their knowledge, the business has no has liens, judgments against it. It is highly recommended to all buyers to conduct a UCC search, background search on seller(s), obtain a good entity standing certificate as part of their due diligence. *
CONSULTANT. The seller can provide the buyer with a list of qualified Medicare consultants that can help the new owner with a Change of Location (CHOL) (as applicable) and the Change of Ownership (CHOW) paperwork.
ALL potential buyers must: (1) Sign an NDA, (2) provide Proof of Funds (redacted), (3) provide a Buyer Profile with a Buyer Representation for this one business and (4) understand that no direct communication with the Seller or the clinical or administrative staff is expected as a part of the pre-due diligence phase of the sale.
COOPERATING BROKERS/AGENTS. We will establish a Co-Op Agreement with licensees, though we do request for all inquiring cooperating licensees to have healthcare business sales experience and knowledge.
